January 2012
In January 2012, the Practice Coaching Program Affinity Group welcomed Kimbra Wells Metz, MHA, Director of Practice Transformation at the Washington Academy of Family Physicians (WAFP), to discuss mechanisms for enabling family physicians to practice at the core of health care. In addition to a discussion of relevant principles and practice models, Kimbra shared information on a future study to compare the roles of practice coaching and multi-payer reimbursement in sustaining practice transformation specific to practices enrolled in a PCMH Collaborative.
February 2012
The Practice Coaching Program Affinity Group meeting in February focused on methods for developing a practice coaching program, specifically in terms of funding and evaluating a program. Lyndee Knox from LA Net discussed the Practice Facilitation Manual recently published by AHRQ, and offered insight from her experiences as Director of LA Net. In addition, the Humboldt Alliance shared with the group a summary and status of the practice coaching program underway in the Humboldt community.
March 2012
In March 2012, the Practice Coaching Program Affinity Group discussed methods for monitoring the progress of practice coaches. Jen Powell, Darren DeWalt and Richard Scoville discussed tools and templates intended to align program key drivers with measurement and evaluation tools. They highlighted the importance of collecting data in ways that maximize the use of monitoring tools, using the data to facilitate discussion and reallocate resources, and creating value at the practice level. In addition, the Cincinnati Alliance shared with the group a summary and status of the practice coaching program underway in the Cincinnati community.
April 2012
The Practice Coaching Program Affinity Group meeting in April reviewed the major themes and challenges emerging across Alliances in their practice coaching programs. Jen Powell shared an assessment of the current status of practice coaching programs in each of the 8 Alliances based on targeted interviews and outreach. The discussion progressed to potential future directions for the PCPAG, which may include in-depth skills-building sessions interspersed with detailed case studies on each Alliances’ practice coaching program. In addition, the Kansas City Alliance shared with the group a summary and status of the practice coaching program underway in the Kansas City community.
June 2012
The Practice Coaching Program Affinity Group meeting in June was the first skills building session developed as part of a new direction for the PCPAG curriculum. The session, entitled An Introduction to Using Clinical Microsystems to Improve Healthcare, was open to all practice coaches working within the participating Alliances. Valerie Jackson, Practice Improvement Advisor, MMC Physician-Hospital Organization, and Lisa Leary, Associate at the Dartmouth Institute Clinical Microsystem Academy, discussed the role of microsystems in practice coaching and provided many tools for practice coaches to use.
July 2012
The Practice Coaching Program Affinity Group meeting in July was focused on a review of the previous month’s discussion of microsystems and the barriers revealed during April’s meeting. Participants shared stories and personal experiences with implementing microsystems in various types of practices. Next month the group will engage in the first in-depth case study of an alliance practice coaching program, highlighting the work occurring in Western New York.
August 2012
The Practice Coaching Program Affinity Group meeting in August was the first in-depth case study of an alliance practice coaching program, highlighting the work occurring in Western New York. The alliance shared the evolution of their practice coaching program, focusing on both the successes and challenges they’ve encountered. Michael Parchman from the MacColl Institute also contributed to the discussion, emphasizing the importance of sustainability and suggestions for achieving it. Next month the group will listen to Peter Margolis, MD, MPH discuss the principles of reliability in primary care.
September 2012
The Practice Coaching Program Affinity Group meeting in September was about reliability science and its application to healthcare. Peter Margolis, MD, MPH from ABMS/IPIP shared the history, definitions, and healthcare applications of the principles of reliability science. Participants gained knowledge on strategies for including reliability principles in project design and coaching methods. Next month the group will learn about Cleveland’s practice coaching program through the second in-depth Alliance case-study.
October 2012
The Practice Coaching Program Affinity Group meeting in October focused on Cleveland’s practice coaching program in the second in-depth Alliance case study. Keith Mandel, MD, Medical Director, and ABMS/IPIP Technical Assistance provider and Gorden Mosser, MD, AF4Q National Advisory Committee were in attendance to provide expert support and advice as needed. Participants were provided detailed knowledge of both the practice coaching program and the history of the Cleveland Alliance. There will not be a call in November due to the National Meeting. The group will reconvene on December 10th and learn about the practice coaching program underway in the South Central Pennsylvania Alliance.
December 2012
The Practice Coaching Program Affinity Group meeting in December focused on South Central Pennsylvania’s practice coaching program in the third in-depth Alliance case study. Keith Mandel, MD, Medical Director and ABMS/IPIP Technical Assistance provider and Michael Parchman, MD, Director at the MacColl Institute were both in attendance to provide expert support and advice as needed. Rush Gross and Stacy Ropp from South Central Pennsylvania provided detailed knowledge about the practice coaching program as well as the Alliance’s maturity index tool. The group discussed various thoughts around segmenting practices into risk categories and ideas for matching practice support resources based on a portfolio of practices and risk levels. The group will reconvene on January 14th for a skills building session.
January 2013
The Practice Coaching Program Affinity Group meeting in January focused on the Humboldt County Alliance’s work with patients in quality improvement through their Patient Partners Project. Jessica Osbourne-Stafnes, Betsy Stapleton, and Sarah Kerr discussed how practice coaches support patients and performance teams as they work together on quality improvement efforts. They shared specific examples of how patient partners interact with the practice coaches through the analysis of case studies. The group discussed thoughts around coordinating with patients throughout the quality improvement process. The affinity group will reconvene on February 25th to continue learning about practice coaching activities underway in Humboldt County.
February 2013
The Practice Coaching Program Affinity Group meeting in February focused on the Humboldt County Alliance's work through an in-depth case study. Rosemary DenOuden and Linda Cade developed the case study which culminated in a set of challenges shared with the group. In response to these challenges, the group discussed the Maine Alliance Service Agreement (SA) and its effectiveness in negotiating an agreement between the practice coach and a practice. Keith Mandel and Jen Powell shared a practice segmentation tool developed by IPIP faculty. They discussed whether there was interest among alliances to prototype the tool. The affinity group will reconvene on March 11th to provide feedback for the IHI Conference presentation, entitled "Moving Patients Along the Continuum of Engagement".
Who You Need to Know
Improving Performance in Practice (IPIP) is a regionally-based, nationally led quality improvement initiative. It brings together technology, data, and proven quality improvement methods to help physicians give their patients the best possible care. The Practice Coaching Working Group includes Alliance leaders across organizations to create a team to spread practice coaching as a method for engaging practices in ambulatory quality improvement activities. Currently five Alliances: Albuquerque, Cleveland, Maine, South Central PA, and Western NY, have created a PCPWG team to drive the development and spread of the coaching program in their community.
