Aligning Forces for Quality is the signature effort of the Robert Wood Johnson Foundation to raise the quality of health care in targeted communities throughout the United States. This community-centric approach to solving problems in our health care system is already providing stories of success, or bright spots, that can serve as springboards for other communities facing similar issues. This can happen only if Bright Spots are readily shared. This is why the AF4Q National Program Office would like to hear about your community’s Bright Spots. The template application is available here.

Applications are due no later than March 2, 2012, to brightspots@forces4quality.org
What You Could Win:
Share your Bright Spot in person at the AF4Q National Meeting in New Orleans this coming May! Selected Bright Spots will be featured in panels or in videos, and if you are invited to come to the meeting, we will cover your expenses. Bright Spots will also be showcased on AF4Q’s website(s) and in other materials.
