You Don’t Find, What You Aren’t Looking For

AF4Q Hospital Quality Network participants are beginning to see results.  The University of New Mexico (UNM) Hospital has been a leader in race and language data collection for years creating a robust program, including interpreters, interpreter services via video and telephone, and translated documents. Through their participation in the Hospital Quality Network, they have started tracking their heart failure readmissions in relation to race, ethnicity and language and have noticed a disproportionate percentage of heart failure readmissions are Hispanic patients.  Now they are working on several initiatives to address this issue, including a cultural integration project so Hispanic patients can better address self-management for their heart failure condition when they are out of the hospital.
