A cross-cutting theme among the Hospital Quality Network focus areas has been improving outcomes and quality of care for all patients. A large part of achieving these aims requires improving the safety and effectiveness of care transitions at all points. Please join the entire Hospital Quality Network for a discussion on evidence-based transitional care models that have improved patient outcomes and reduced costs as well as an overview of opportunities available to hospitals from CMS.
Traci Archibald, OTR/L, MBA, will present key findings and lessons learned from CMS' Care Transitions Theme, highlighting the efforts of QIOs in 14 states who are working to promote seamless transitions from the hospital to home, skilled nursing care or home health care. Juliana Tiongson, MPH, will give an overview of Section 3026 of the Affordable Care Act, modeled after the work of the 14 QIOs and present the new Community-Based Care Transitions Program (CCTP). She will also discuss the ways in which AF4Q Communities and HQN hospitals can take full advantage of the new and timely Partnership for Patients opportunities.
