The keynote speaker for our January 18th Quality Forum is Darren DeWalt, MD, MPH, from the University of North Carolina at Chapel Hill, who will discuss his research on health literacy and heart failure patient education. As part of this webinar, we would like to spotlight some of the heart failure materials the Reducing Readmissions hospitals are using to educate patients and families about heart failure and help them self-manage their condition. 

We continue to accept the heart failure educational materials your hospital most consistently gives to your patients/families to hqn-readmissions@forces4quality.org. Include a brief description of how these tools are used at your hospital. We will feature these education materials on the Hospital Quality Network: Reducing Readmissions website.
