Project Healthy Grad
West Michigan’s Project Healthy Grad AF4Q PHP initiative aims to increase high school graduation rates, while ensuring all high school students are prepared for and have access to post-secondary education. West Michigan will work to require schools to screen for student insurance status and eligibility, connect students to tuition incentive programs based on Medicaid/S-CHIP eligibility, and explore changing Medicaid administrative rules to obtain additional revenue for schools to implement this screening. Deliverables will include new policies requiring schools to implement these insurance identification and eligibility screening activities and provide enrollment services for Medicaid/S-CHIP if eligible, changing the screening process for a state-funded tuition incentive program based on Medicaid/S-CHIP eligibility, and creating additional revenue to support Community Health Workers in schools.
