The Institute for Patient- and Family-Centered Care (PFCC) put together partnership tips and tools for practice coaches in PCMH transformations. These outlined the role of QI specialists and other practice facilitators; assessed practice readiness to engage patients and families; explained how to engage leadership sponsorship and buy-in; highlighted tools and strategies for practice coaches; explained how patient experience of care data can be utilized by advisors and practices; and discussed a roadmap that identifies initiation, improvement, and sustainability efforts.
