The Practice Coaching Program Manual was reviewed by 16 quality improvement professionals, including IPIP State Directors, experienced practice coaches from across the county, IHI faculty, and the MacColl Institute. We gratefully acknowledge their expertise and time in assisting us to provide a resource as you develop, maintain and spread your practice coaching program. The Manual was updated in October of 2013 to reflect the changes in the field and the innovative work of the AF4Q Alliances.
