This documents depicts how physician organizations and practices, in the phase of implementing Patient-Centered Medical Homes (PCMH), are involving patients and families in quality improvement and redesign efforts. This document explains what patient- and family-centered care is and how it leads to patient engagement at all levels. It also shows how the involvement of patients and families has yielded several benefits and insights.
