Reducing Readmission's July Quality Forum focused on improving care across settings.  Dr. Eric Coleman, from the University of Colorado at Denver, is the Director of the Care Transitions Program and works to improve quality and safety during times of care "hand offs".  In this month's Quality Forum, he discussed the Care Transitions Intervention model, it's impact on improving the continuum of care for heart failure patients, and lessons learned from hospitals that have adopted the model.
