The Care Model is a population-based model that relies on knowing which patients have the illness, assuring that they receive evidence-based care, and actively aiding them to participate in their own care. The model is was designed to help improve health disparities at a primary care practice level and is a great resource for Ambulatory Quality Improvement Initiatives. The major objectives of the six elements of the Chronic Care Model represent a principle for redesigning care.
