Implementing Patient-Centered Medical Home (PCMH) Pilot Projects: Lessons from AF4Q Communities

As the patient-centered medical home (PCMH) model emerges as a key vehicle to improve the quality of healthcare and to control costs, many AF4Q communities are implementing pilot programs that promise to reinvigorate primary care as the driver of larger changes within regions.

For AF4Q communities, patient-centered medical home initiatives promise to move the dial on key dashboard indicators, enabling consumer-driven care, activating patient engagement, rationalizing and coordinating processes and ultimately improving outcomes.

This update from the Ambulatory Quality Network highlights the implementation of PCMH pilot projects across numerous alliances. The document summarizes common themes found throughout the projects, including necessities for success as well as barriers to overcome. Snapshots from three participating communities (Cincinnati, Maine and Western New York) are provided to give a more in-depth look at development, implementation and adoption of these programs.
