Hospitals use AF4Q heart failure calendar to improve care: 2012 edition just released!

The 2012 Heart Failure calendar and companion nurse teaching guide are now available to help heart failure patients to take control of their condition. You Can Live with Heart Failure – Healthy Habits for Life is available in both English and Spanish. With areas to record information, patients use the calendar to record their weight and track their heart failure symptoms. Healthcare providers can use it to educate heart failure patients about their condition and teach them self-management skills. Hospitals can customize the calendars by adding their hospital logo to the front cover. The calendar and teaching guide were originally developed by a special workgroup of nurses from the AF4Q Expecting Success collaborative.
