Better Health Greater Cleveland, a program of the Robert Wood Johnson Foundation's Aligning Forces for Quality initiative, focuses on improving care by measuring and improving patient-centered primary care for its residents with chronic medical conditions. Focusing primarily on Cuyahoga County, Ohio, Better Health has expanded the number of clinical members in the county, that allows them to better serve their target population.

 

The results of Better Health's 10th Community Health Checkup are highlighted in this executive summary. This report describes the care and outcomes of 137,600 unique patients at 55 practice sites of eight health systems receiving care from 614 providers, which include general internal medicine, family practice and medicine/pediatrics physicians and advanced practice nurses with prescribing privileges.

Cuyahoga County was successful in lowering costs of hospitalizations. They reported nearly 3,000 fewer ambulatory care sensitive hospitalizations between 2009 and 2011 (a cost savings of $20 million.)

 

Improved County Health Rankings--Cuyahoga County ranks 65th out of 88 Ohio counties.
